    Dupilumab improves congenital ichthyosis in the context of a filaggrin pathogenic variant. This report from Germany confirms other reports of a response of rare ichthyotic conditions such as Netherton’s syndrome and trichothiodystrophy to targeting IL-4Rα, and that the common FLG mutations may accentuate the inflammatory phenotype of the inherited ichthyoses.

    Expression of phosphorylated-Janus kinase 1 and IL-4Rα by dermal basophils and epidermal keratinocytes in atopic dermatitis-associated prurigo nodules. This well-researched Japanese case report highlight with conventional histopathology and immunocytochemistry that the target of baricitinib in prurigo nodularis could be phosphorylated JAK1 expressed mainly by basophils and epidermal keratinocytes.

    Adding Fuel to the Fire? The Skin Microbiome in Atopic Dermatitis. This is a helpful review in a complex field with a warning on untimely intervention.  Skin microbiome studies in AD have shown that disease flares are associated with microbial shifts, particularly Staphylococcus aureus predominance. But AD-associated S. aureus strains differ from those in healthy individuals, and we need first to understand better the complex microbiome – barrier- immune system interactions in AD to propose useful interventions.
